This course provides an introduction to chronic disease management and the role of case management in addiction services. Twelve core functions in substance abuse counseling are explained. Participants learn how to develop a treatment plan, match clients to appropriate community services, make successful referrals, increase motivation for adherence, and coordinate continuing care. Procedures for outcome monitoring and program evaluation are reviewed. Case closing and termination procedures are discussed.
